In 2014 the Lower Hudson Valley Perinatal Network received funding from New York State Department of Health for the Hudson Valley Connect (HV Connect) project. This project will further LHVPN’s work to enable electronic referrals and data exchange between medical and human services agencies in the region. HV Connect will build upon LHVPN’s previous work in providing electronic prenatal appointments to be made for women in a variety of social service settings and electronic referrals and exchange of patient and client information between medical providers via the PeerPlace Network. HV Connect will link the regional PeerPlace Network with the local regional health information organization (RHIO), the Taconic Health Information Network and Community (THINC). Connecting these two systems will mean that providers will be able to share information about clients including medical documents such as laboratory test results and ultrasound pictures and pertinent information for social service provides such as the reasons for needing home visiting or other services. Additionally, community health workers will be able to use PeerPlace to maintain their clients’ records.
