Who is It’s About You for?
It’s About You! is for women and their families who desire to live a healthy lifestyle. Focusing on improving the health of men and women in the communities, whether they are expecting a child or not, leads to healthier neighborhoods.
What can It’s About You do for my family?
The It’s About You program helps you steer your life in the direction of your choice. Individuals are self-empowered to take control of their health, mind, body and environment.
How does It’s About You work?
Services are voluntary and provided at no cost by skilled Community Health Workers, trusted members of the community, who are committed to supporting you in achieving your personal goals. The primary goal of this partnership is to provide you and your family with support and resources connected to health care services, family planning benefits, and community services that help stabilize families.
What types of services do you provide?
Services include connecting you and your family to health insurance; family planning benefits; securing a medical home for you, your child or family where you can receive healthcare services inclusive of mental and dental health; in-home services (home visits); access to educational and vocational training; financial literacy support (budgeting and saving); and community services that support safe and healthy personal relationships.
